What to Expect at Your Medical Appointment

Due to insurance requirements, the physicians must see patients for a consultation in order to move forward with procedures or treatments. The timeframe will vary based on the physician’s opinion of the recommended course of treatment and the patient’s insurance coverage. At times, insurance approval can take up to 90 days from the date of the consultation and/or required tests (visual field tests, CTs, MRIs, etc.). For your medical consultation, we require the following:

  • Copies of Photo ID and all Insurance cards
  • List of current medications
  • Referral from your physician with authorization from your insurance
Payment for appointments will be based on your individual insurance contract. You will likely have a specialists copay due at the time of the appointment. Check with your insurance company for details. We require a credit card to hold your appointment for consultations. The card will only be charged to patients who do not provide a 24-hour notice of cancellation or rescheduling. Due to a large demand for appointments and that we are a specialty, it’s important that we will appointments as efficiently as possible.
