At times, insurance approval can take up to 90 days from the date of the consultation and/or required tests (visual field tests, CTs, MRIs, etc.). • List of current medications
  • Referral from your physician with authorization from your insurance
Payment for appointments will be based on your individual insurance contract. You will likely have a specialists copay due at the time of the appointment. Check with your insurance company for details. We require a credit card to hold your appointment for consultations. The card will only be charged to patients who do not provide a 24-hour notice of cancellation or rescheduling. Due to a large demand for appointments and that we are a specialty, it’s important that we will appointments as efficiently as possible.
